Hi @Harej: and thanks for this request and COMPLETELY APOLOGIES if nobody noticed your request.

I've added a small ping to myself in the form template, to introduce an automatic notification to myself when new requests are submitted, to be more proactive in finding this kind of very appreciated form requests, to escape from Google Forms.

I hope that this will not repeat again.
