Parameters

1: Username.
timezone_1: freetext field for timezone information Optional
lang_1: freetext field for languages information Optional
2 through 5: Same as 1
Date: "y" will insert the phrase "This note was updated on" followed by the current month and year when the template was last edited. Optional
