Ombuds commission/2021 workflow updates/Form

Similar with Stewards' contact form, it is proposed that the Ombuds Commission start using a form to receive complaints from community. With that form, it's expected that the commission will have more organized and straightforward messages.

It's going to be used the Stewards' contact form as a startpoint, but the plan is to have one more adequate to our needs.

    • The account Ombuds commission was created for that purpose (mail address is the Ombuds’ list).
    • Messages will keep being sent to the OC mailing list (cu-ombuds-l lists wikimedia org)

Header edit

There's no special need on the header. Username, mail address and subject should all be required.


Settings edit

The proposed fields were designed in order to channel the explanation of the cases to what is fundamental for their understanding. The Stewards' contact form has only one field (Message). Below, it's being proposed separate fields to be optionally filled.


  • Case explanation (required)
    • Instruction: Explain what has happened.
  • Relevant links (optional)
    • Instruction: Provide URLs to the wiki actions or pages.
  • What privacy violation or tool misuse has taken place? (Required)
    • Instruction: In a few words, explain what wiki policy was violated.
  • Involved users (optional)
    • Instruction: What users are you complaining about? Provide their wiki usernames.
  • Affected accounts (optional)
    • Instruction: Which accounts are affected by this complaint? If nobody else than you was affected, you can skip this field.
  • Proposed solution (optional)
    • Instruction: What solution would you propose? What are your expectations from the commission?
  • Additional information (optional)
    • Add any other information that you believe to be relevant for the understanding of this case.
